We are a growing patient-centered practice that is looking for a full-time Community Based Rehabilitation Specialist with our Family Support Services team in Coeur d'Alene, Idaho.


This position available would support our ACT (Assertive Community Treatment) team. We are looking for independent individuals who want to make an impact in others' lives! This individual would be working full time, Wednesday through Sunday.

The Community Based Rehabilitation Specialist (CBRS) works individually with Heritage Health clients who are in the mental health program to help them build life skills and work towards a place of recovery. Job duties are performed in the client's home or in their community. CBRS providers work closely with other members of the client’s supports and services team to assist clients in meeting their rehabilitation and mental wellness goals.


In this role, you will:
  • Assist clients and supports to develop treatment goals and objectives.
  • Provide appropriate mental health interventions designed to help clients develop skills that will enable them to reach recovery as indicated in Service Plan.
  • Maintain electronic client files, including timely completion of progress notes, services plans, and reviews.
  • Utilize Microsoft office programs and email daily.
  • Regularly attend staff meetings and supervision.
  • Comply with confidentiality policy as it applies to protection of clients’ rights.
  • Communicate and collaborate with all members of the treatment team.
  • Comply with CBRS Rules and Regulations.
  • Follow all mandated reporting laws.
  • Strictly adhere to Professional Code of Ethics for appropriate discipline (if not aware of this code, must obtain copy and become familiar with it)
  • Provide all services in a professional and respectful manner at all times
  • Understand and adhere to all Heritage Health / Family Support Services Policies and Procedures.
  • Travel to client’s homes and other locations in the community daily.
  • Other duties as assigned by supervisor(s)
Qualifications:
  • The position requires a minimum of a BA/BS in a Behavioral Science
  • Experience in the field of mental health
  • Attainment of either the CPRP, CFRP or LSW within 30 months of employment 
  • Reliable transportation required.
