Description
The mission of Heritage Health is to deliver a healthcare experience that provides hope, inspires change and extends life for our patients and our community.  We are a growing patient-centered practice who is looking for a Mental Health/Substance Use Disorder Counselor in St. Maries.
 

The Counselor is responsible for providing confidential addiction counseling, education and support to individuals, families and the community and for promoting healthy lifestyles and healthy choices.  Performs psychosocial assessments and provides ongoing direct social work services to patients and families.  Identifies unmet psychosocial needs and develops programs to address those needs.


Minimum eligibility and qualifications:  

LMSW or LCSW license in the State of Idaho; Master’s degree or higher in human service field with post graduate experiences and clinical endorsement; working knowledge of Administrative procedure of a Federally funded community health center; computer skills necessary to maintain client charts and communication within the organization; excellent communication skills.
 

Duties and Responsibilities:
  1. Counsel clients or patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
  2. Formulate and document in the medical record an individualized plan of care for patient in a timely manner, in compliance with departmental guidelines.
  3. Provide a wide range of social work services, i.e., direct counseling services, resource management, etc. in accordance with departmental guidelines.
  4. Complete and maintain accurate records or reports regarding the patients' histories and progress, services provided, or other required information.
  5. Develop client treatment plans based on research, clinical experience, and client histories.
  6. Coordinate counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.
  7. Interview clients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program.
  8. Review and evaluate clients' progress in relation to measurable goals described in treatment and care plans.
  9. Provide clients or family members with information about addiction issues and about available services or programs, making appropriate referrals when necessary.
  10. Coordinate activities with courts, probation officers, community services, or other post-treatment agencies.
  11. Compliance with documentation standards set by the state and federal guidelines.
  12. Knowledge and use of evidence-based best practice therapeutic models.
  13. Role modeling, organizational, communication and guest-relation skills.
  14. Willingness to attend and participate in off-site meetings and/or seminars.
  15. Complete assessments.
  16. Other duties as assigned.
