The mission of Heritage Health is to deliver a healthcare experience that provides hope, inspires change and extends life for our patients and our community. We are a growing patient-centered practice who is looking for a full-time Community Based Rehabilitation Specialist in Coeur d' Alene.
 

Community Based Rehabilitation Specialist (CBRS) works individually with Heritage Health client’s who are in the mental health program to help them build life skills and work towards a place of recovery. Job duties are performed in the client's home or in their community. CBRS providers work closely with other members of the client’s supports and services team to assist clients in meeting their rehabilitation and mental wellness goals.

Minimum eligibility and qualifications:
The position requires a minimum of a BA/BS in a Behavioral Science and experience in the field of mental health, and attainment of either the CPRP within 30 months or Children’s Certificate within 12 months of employment. Reliable transportation required.


Duties and Responsibilities:

  1. Assist clients and supports to develop treatment goals and objectives.
  2. Provide appropriate mental health interventions designed to help clients develop skills that will enable them to reach recovery as indicated in Service Plan.
  3. Maintain electronic client files, including timely completion of progress notes, services plans, and reviews.
  4. Utilize Microsoft office programs and email daily.
  5. Regularly attend staff meetings and supervision.
  6. Comply with confidentiality policy as it applies to protection of clients’ rights.
  7. Communicate and collaborate with all members of the treatment team.
  8. Comply with CBRS Rules and Regulations.
  9. Follow all mandated reporting laws.
  10. Strictly adhere to Professional Code of Ethics for appropriate discipline (if not aware of this code, must obtain copy and become familiar with it)
  11. Provide all services in a professional and respectful manner at all times
  12. Understand and adhere to all Heritage Health / Family Support Services Policies and Procedures.
  13. Travel to client’s homes and other locations in the community daily.
  14. Other duties as assigned by supervisor(s)
